This report describes the processes and outcomes of the third year of a 7-year quality improvement project that used educational processes - Academic Detailing & Practice Facilitation - to assist primary care practices serving disadvantaged populations ("safety-net practices") in the implementation of evidence-based interventions to improve screening rates for colorectal, breast, and cervical cancer. The overall structure of the project was for three academic departments to serve as guides and resources for the safety-net practices. The project was geographically focused in central and western New York State (Syracuse, Rochester, & Buffalo, NY). The report describes interventions that can be used by other academic departments to assist community partners (such as preceptor offices) in implemented quality improvement in cancer screening. Additionally, the ideas and concepts may be used by practices themselves, by residencies, or other clinicians or academics to drive cancer screening quality improvement on their own.A version of this report was originally submitted to the New York State Department of Health.
